MVMT Evanston is in network with Blue Cross Blue Shield PPO.

It is highly recommended that you call the number on the back of your insurance card to also check your physical therapy benefits. Typically, folks will have a deductible (anywhere from $500-$5000) to meet prior to a percent based insurance coverage or co pay.

self pay:

I offer a cash based service for folks with other insurance carriers.

$150 for an evaluation and follow up appointments.  1 hour, one on one care.

Upon request, I can provide a superbill for you to submit to your insurance to be applied towards your out of network deductible.
